Registered Nurse
Will be certified to become a Medication Administration Trainer through the Kentucky state DDID Medication Administration RN Direct Trainer training.
Will be required to take a medication curriculum through the College of Direct Support (CDS).
Must have a current RN license and be willing to sign a written contract/agreement with My Life Solutions to provide Medication
Administration training for all of My Life Solutions employees.
$38.50 per hour (Contract)
Direct Support Professional (DSP)
The Direct Support Professional (DSP) provides direct, person-centered services to clients receiving Supports for Community Living (SCL) services. DSPs assist individuals in achieving their personal goals, independence, and community inclusion, while ensuring their health, safety, and well-being.
DSPs are responsible for implementing supports as outlined in each individual’s Person-Centered Service Plan (PCSP) and providing care consistent with Kentucky SCL regulations and My Life Solutions policies.
Must have a high school diploma or GED and at least two (2) years of experience working in a healthcare setting.
Additional Requirements: current CPR/First Aid certification, current negative TB results, must provide current dmv record, valid driver's license, and proof of auto insurance.
$18 per hour
Director of Admissions
The Director of Admissions is responsible for overseeing the intake, assessment, admission, and discharge processes for individuals served by My Life Solutions. This position ensures that individuals are appropriately referred, evaluated, and matched to services in accordance with Kentucky Supports for Community Living (SCL) requirements and person-centered practices.
The Director of Admissions also provides supervision to Community Access Specialists and Family Home Providers (FHPs) and ensures that services are delivered in a manner that supports individual choice, health, safety, and successful community integration.
Bachelor's degree in healthcare administration or business preferred or an equivalent combination of education and experience. Must have at least two (2) years of experience working in a healthcare setting.
Additional Requirements: current CPR/First Aid certification, current negative TB results, clean drivers motor vehicle records-3 years, valid driver's license, and proof of auto insurance.
$30 per hour (Contract)
Family Home Provider (FHP)
FHPs provide direct, person-centered services to clients receiving Supports for Community Living (SCL) services, allowing the client to live in their personal home with them and their family. FHPs assist clients in achieving their personal goals, independence, and community inclusion, while ensuring their health, safety, and well-being.
FHPs are responsible for implementing supports as outlined in each individual’s Person-Centered Service Plan (PCSP) and providing care consistent with Kentucky SCL regulations and My Life Solutions policies.
Must have a high school diploma or GED and at least two (2) years of experience working in a healthcare setting. Must have a home with a private room and access to a shared bathroom.

$90 per day per client (max 3 clients per home) (Contract)
Program Services Director
The Program Services Director is responsible for the overall oversight, coordination, and quality management of Residential Level 1 services at My Life Solutions. This role ensures that all homes operate in compliance with Kentucky Supports for Community Living (SCL) regulations and provide safe, person-centered, and high-quality supports to clients served.
The Program Services Director provides direct supervision to Home Managers and ensures consistent implementation of Person-Centered Service Plans (PCSPs), staff training, health and safety standards, and regulatory compliance across all assigned residential
settings.

Community Access Specialist
The Community Access Specialist provides direct, person-centered supports to clients
receiving Supports for Community Living (SCL) services, with a focus on community engagement, skill development, and independence.
This role assists clients in accessing and participating in meaningful community activities, developing social and daily living skills, and achieving goals outlined in their Person-Centered Plan Service (PCSP). Services are delivered in accordance with Kentucky SCL regulations, emphasizing health, safety, dignity, and community inclusion.
Must have reliable transportation, a high school diploma or GED, and a flexible schedule. Must have at least two (2) years of experience working in a healthcare setting.
